Shockwave, or focused ultrasound stimulates the body’s natural self-healing process. Following treatment of an injury site, there is often reduction of pain and improved range of motion. Typically, shockwave therapy is used to promote accelerated tissue repair and joint healing, analgesia (pain relief) and improved joint mobility. Shockwave may also eliminate your need for surgery depending on the amount of instability present in the injured area. It can help to heal ligament and muscle tears smaller than 3mm, and can help chronically injured areas begin a new healing phase.

The number of treatments and use of different shockwave instruments depends on the severity, chronicity and location(s) of the condition(s).
Normally four to twelve treatments are necessary at weekly or biweekly intervals; there is a small possibility that additional treatments may be necessary if your condition is very chronic and/or you have severely restricted joint range of motion (ie. Frozen shoulder).
Should you not respond during this time, your case will be reviewed with the doctor to determine an appropriate referral. Success rates with shockwave are unparalleled (over 80-90% of patients see long term improvement).
Initially, as the shockwave engages injured tissue or ischemic areas i.e. sites with low blood flow or scar tissue, the initial treatment may be uncomfortable. However, most people can easily tolerate the procedure.
There may or may not be pain or soreness after treatment, but some discomfort may be experienced 2-4 hours after the treatment. This treatment does break down scar tissue, and the body does need some time to heal. In some cases, it can last up to 48 hours and in very rare cases, the pain or soreness has lasted up to 5 days. Some bruising and swelling can occur, although it is very rare.
Shockwave will trigger an inflammatory response, which is the body’s natural process of healing. For this reason, do not use anti-inflammatory medications. You can use ice in a limited fashion, no more than 20 minutes per hour. The pain should subside within 24 hours.
Even if it feels good, we recommend decreased activity for 48 hours following the treatment. Your tissue is healing, and overuse can lead to reinjury.
Although FDA approved, Shockwave is not covered by insurance.
A successful treatment is considered as a patient having at least 75% reduction in pain within 3 months with improvements in pain free range of motion, strength, and improved functional ability. Worldwide, success rates with shockwave have been reported at 80 to 90%!
Although many improvements are noticed in the first few weeks of treatment, the long-term benefits of this treatment may take up to 3-4 months to see maximum improvement, and in some rarer cases 6-12 months. If after this time there has not been any marked improvement, you should see your doctor for further treatment options.
Contraindications include coagulation disorders, thrombosis, patients using anticoagulants, especially marcumar, heparin, and coumadin. Cancer patients, polyneuropathy in patients with diabetes mellitus, sites with pus in the target area.
There are no known major side effects.
Some minor side effects may include: (These side effects are rare and generally abate after 2-5 days). • Swelling, reddening, hematomas, petechiae, bruising, pain.
